3 Smiling Hearts Home Healthcare LLC are here to provide private duty care to patients of all ages. This service is provided in the comforts of the client's home.

3 Smiling Hearts Home Healthcare offer home care services to patients so they may remain safe in the comforts of their home. We serve clients of all ages. This service is available 24 hours a day, 7 days a week, and for as long as the client and his or her family agrees to participate in the plan of care.

CDS

A Missouri Medicaid-based program which can help beneficiaries hire and train caregivers of their preference. This is a great alternative to traditional

in-home services.

Consumer Directed Services (CDS) is a consumer-managed program available for individuals who are Missouri Medicaid eligible. This type of program allows the consumer to employ and train caregivers of their choice; be it a family member, relative, or friend, with the exception of a spouse or anyone under 18. The caregivers are to serve the consumer with the same services given in a traditional in-home care service. As such, these chosen caregivers can be paid for their service.

To qualify for consumer directed services (CDS), a person must be:

Medicaid eligible (MO HealthNet eligible)
18 years old or above
Able to direct/supervise their own care
Physically disabled
Capable of living independently with the services of CDS
Requires at least a nursing facility level of care

IN-HOME

This type of program is applicable for senior or disabled patients who are not capable of employing, training, and directing their own caregiver of choice.

In-Home Services (IHS) is a Medicaid-based program which provides home care services to individuals who are in need of assistance in their

personal tasks at home. Consumers of this program are not enabled to manage their own care and are not responsible for searching, hiring, and training their caregivers. Instead, 3 Smiling Hearts Home Healthcare will handle all of this process for the client.
